The form below is used to register each individual camper as well as counselor. If the person you are registering is a counselor in the grade field you will want to indicate that by typing in " Jr. " if they are a Junior counselor and " Teen " if they are a teens counselor.

IMPORTANT: This form will need to be filled out for each individual camper attending Mount Zion and should only be filled out by either the Youth Pastor or Camp Counselor.

Tip #1: Don't retype Church and Pastor's name every time try pressing the down arrow on the keyboard. This can also be used for city, state, and zip.

Tip #2: Use the tab key on the keyboard to jump to the next form field.
